(hereinafter, the “Trainee”) desires to attend the nurse aide training program administered by Absolute Health Services. (“Absolute”) that is intended to provide the Trainee with the training and education required by the State of Ohio as one of the prerequisites for becoming a State Tested Nurse Aide (STNA) or a home health aide (HHA). Hereinafter, the “Program”.

In consideration of Absolute accepting the Trainee into the Program, the Trainee agrees to, and acknowledges understanding of the following:

  1. PROGRAM ENROLLMENT FEE: The cost of enrollment in the Program classes is $455.00 (“Program Enrollment Fee”). Trainee agrees to pay at least ½ of the Program Enrollment Fee in advance and in full by the 6th day. If for any reason the Trainee does not complete the class, payment in full will be required. This fee does not include the cost of state testing. All payments must be in the form of a money order, Visa, MasterCard, a bank-issued cashier’s check, or cash.
  2. SCHEDULE: The Program shall be conducted in accordance with the published schedule that shall be provided to the Trainee. Absolute reserves the right to reschedule Program sessions at its sole discretion.
  3. TUBERCULOSIS TEST: Trainee agrees to submit to a tuberculosis test during the course of the Program. The test shall result in a negative reading. If there is a positive reading, a physician’s statement must show that treatment is being provided or that treatment is contraindicated. A copy of the Trainee’s tuberculosis test must be provided to the Program. Only a one-step tuberculosis reading is needed.
  4. COMPLIANCE WITH POLICIES AND PROCEDURES: Trainee agrees at all times to abide by all of the policies and procedures of the Program and of Absolute.
  5. EXPULSION FROM THE PROGRAM: Absolute reserves the right to bar the Trainee from attending the Program if, in the sole discretion of the Program Director, the Trainee fails to substantially comply with the policies and procedures of the Program or of Absolute, or violates any laws or regulations relating to patient safety. Under such circumstances, the Trainee shall not be entitled to a refund of the Program Enrollment Fee.
  6. CRIMINAL RECORDS CHECK: Although a records check is not required for the Program, when a Trainee applies for a job one will be required by that facility or agency seeking to hire Trainee. Felonies and offenses listed in OAC 3701-13-05 will potentially disqualify Trainee from employment.
  7. To successfully complete this class and the state test it is imperative that Trainee can read, speak, comprehend and write in English. If Trainee cannot complete the Program due to your failure to do so, payment in full will be expected.
  8. UNIFORMS: It is required that medical scrubs and clinical shoes or tennis shoes are worn each day. Scrubs may be of any color except white.
  9. ATTENDANCE: The first 16 hours of class are mandatory. There are no exceptions. Other absences (maximum of 3) need to be made up within 60 days of missed date. One make up day is at no charge. Remaining make up days are charged $25.00 per missed day. Late arrival and leaving early will be monitored and those hours may result in make up time.
